The Care Transformation Collaborative of Rhode Island (CTC-RI) is seeking applications for up to two Integrated Behavioral Health (IBH) clinicians to become certified as IBH Practice Facilitators.
The Team-Based Care Learning Collaborative is a 10-month participatory learning experience offered by the National Cooperative Agreement for Clinical Workforce Development, funded by the Health Resources and Services Administration, and hosted by Community Health Center, Inc.
We are grateful for the ongoing work of our board of directors, staff, primary care team members, behavioral health clinicians, community health workers, health plan partners, and state partners for their work to make 2017 a year of progress for our Collaborative.

CTC-RI and the pediatric patient-centered medical homes (PCMH-Kids) practices are excited to be partnering with Onpoint Health Data to support the transformation of primary care in the state of Rhode Island through analysis and reform efforts designed to improve the quality of life, patient experience, affordability of care, and overall health.
Over 300 people attended the 2017 CTC Annual Learning Collaborative which featured national, regional, and local experts speaking on topics to assist primary care practices with using practical and evidence based approaches to improve health outcomes through better utilization of team based care and patient activation strategies.
