Health plans support PCMH-Kids practices
PCMH-Kids and CTC are pleased to announce that all the health plans have agreed to provide continued financial support for PCMH-Kids Cohort 1 practices. The commercial plans will provide infrastructure support through the OHIC sustainability payment plan and the Managed Medicaid plans will provide bridge funding and work with EOHHS to include continued infrastructure support for PCMH-Kids through its 2019-20 rate structure. This health plan investment in children's health is needed and appreciated and we work together to continue to build a strong foundation for children's health.
PCMH-Kids Cohort 1 started in January 2016 with nine pediatric primary care practices serving 30,000 children under the age of 18. This multi-payer, multi-provider primary care payment and delivery system reform initiative has focused on the unique needs of children and families. Over the last three years, the health plans have financially supported and engaged with PCMH-Kids practices under common agreements built around practices progressively implementing service delivery requirements and becoming patient-centered medical homes.
