Tufts Health Plan grants funding support to CTC for 2019

CTC is pleased to announce it has received generous funding from Tufts Health Plan for 2019 to support the following two efforts:

  • PCMH-Kids - working to enhance pediatric practice linkage to the home visiting program
  • Integrated Behavioral Health - developing an online training for IBH practice facilitators, and supporting enhancements to data reporting and business intelligence analytics for the CTC practice portal.

Thanks to last year's funding from Tufts Health Plan, among many critical efforts and programs, CTC was able to host an engaging and successful postpartum depression learning collaborative, and improve PCMH-Kids postpartum depression screening and referral relationships. In addition, thanks to previous funding, PCMH-Kids launched a successful adolescent SBIRT Initiative and CTC's Integrated Behavioral Health program was made possible.
