Congratulations to Patricia Flanagan, MD, FAAP and Elizabeth Lange, MD, FAAP, both of PCMH-Kids -- the pediatric arm of CTC -- who received the 2018 AAP Calvin C.J. Sia Community Pediatrics Medical Home Leadership and Advocacy Award!
Jointly provided by CTC and Brown University Medical School, our CTC Annual Conference held on November 1st to a sold out room of more than 330 attendees! We would like to thank all who attended to discuss and learn ways in which local primary care teams can identify, develop, and utilize comprehensive strategies to improve care and lower costs.
The Care Transformation Collaborative of Rhode Island has worked with more than 100 pediatric, family medicine, and adult primary care practices throughout Rhode Island. Through this work, we have established a track record of health care transformation success, and are now excited to offer help to other organizations desiring specific goals.
The mission of the Care Transformation Collaborative of Rhode Island is to lead the transformation of primary care in Rhode Island in the context of an integrated health care system; and to improve the quality of care, the patient experience of care, the affordability of care, and the health of the populations we serve.
