Congratulations to Patricia Flanagan, MD, FAAP and Elizabeth Lange, MD, FAAP, both of PCMH-Kids -- the pediatric arm of CTC -- who received the 2018 AAP Calvin C.J. Sia Community Pediatrics Medical Home Leadership and Advocacy Award!
Jointly provided by CTC and Brown University Medical School, our CTC Annual Conference held on November 1st to a sold out room of more than 330 attendees! We would like to thank all who attended to discuss and learn ways in which local primary care teams can identify, develop, and utilize comprehensive strategies to improve care and lower costs.
The Care Transformation Collaborative of Rhode Island has worked with more than 100 pediatric, family medicine, and adult primary care practices throughout Rhode Island. Through this work, we have established a track record of health care transformation success, and are now excited to offer help to other organizations desiring specific goals.
The mission of CTC-RI is to support the continuing transformation of primary care in Rhode Island as the foundation of an ever-improving integrated, accessible, affordable, and equitable health care system. CTC-RI brings together critical stakeholders to implement, evaluate and spread effective multi-payer models to deliver, pay for and sustain high-quality, comprehensive, accountable primary care.
