2024 was a pivotal year for primary care in Rhode Island. As our state confronts a historic workforce crisis, the Care Transformation Collaborative of RI (CTC-RI) remains steadfast in its mission to strengthen and sustain primary care for all Rhode Islanders.
Across every community, primary care providers are navigating workforce shortages, rising administrative demands, and the need for better care coordination. Yet amid these challenges, collaboration is driving progress. CTC-RI is convening statewide partners to identify shared solutions, including expanding training pipelines, reducing administrative burden, and supporting innovations that make care more efficient and rewarding for both patients and providers.
From team-based care models and behavioral health integration to the expansion of opportunities for nurse practitioners, nurse care managers, physicians, pharmacists, behavioral health clinicians, and community health workers, CTC-RI is helping to shape a stronger, more resilient primary care system. Through PCMH-Kids, we are also ensuring the next generation of pediatric clinicians is prepared to deliver high-quality, coordinated care that supports families and children at every stage of development.
Rhode Island’s path forward depends on sustained investment and collaboration. Together, we are charting a path for primary care.
