CTC-RI/PCMH Kids Hosts Practice Reporting / Transformation Committee Meeting with Salute to PCMH Kids Cohort 3 Practices

PCMH Kids Cohort 3 practice performance was highlighted at the May Practice Reporting and Transformation meeting including the clinical quality measures and CAHPS customer experience survey results. Practices shared results from their performance improvement plans, which focused on improving adolescent well-child visits, team huddles, screening for lead and anxiety.

Medicaid Pediatric Healthcare Recovery Program Behavioral Health Technical Assistance Learning Sessions

May’s Behavioral Health Technical Assistance learning session focused on Behavior Plan Basics, where participants learned about the core elements of effective behavior plans as well as common pitfalls and why behavior plans fail. Sample behavior plans and behavior contracts that support healthy routines in older children were shared (e.g. sleep, managing electronics).
