In May, CTC-RI hosted the final quarterly learning collaborative for the Pharmacy Quality Improvement Initiative with 46 participants attending this virtual learning collaborative session.
With support from the American Academy of Pediatrics (AAP), CTC-RI received training in the ECHO model learning framework.
In May, CTC’s Clinical Strategy Committee Meeting hosted Ann Greiner, President and Chief Executive Officer of the Primary Care Collaborative.
In a stimulating June Breakfast of Champions meeting, CTC-RI hosted nearly 70 health care leaders to address diabetes from patient/clinical and community/clinical perspectives. Presentations featured diabetes management guidelines, a practice-based pharmacy-led intervention, and a collaborative effort to integrate clinical and community support for disease management.
With the support of the Rhode Island Department of Health (RIDOH) and Tufts Health Plan, pediatric medical homes and providers from the family visiting program continue to improve communication and care coordination with a specific focus on improving well-child care.
