Important updates were shared at the April Best Practice Sharing Committee meetings. Practice clinical quality and customer experience performance was reviewed at the April Practice Reporting and Transformation Committee meeting. Practices are encouraged to review their 1st quarter 2019 clinical quality performance report for accuracy.
Integra Community Care Network, an ACO in partnership with Care New England, RI PCPC and South County Health, has engaged CTC services for a two-year contract to assist selected Integra practices with implementing fully-integrated care that will address the medical, behavioral, and substance use needs of patients in a coordinated, systemic, and unified way.
The mission of CTC-RI is to support the continuing transformation of primary care in Rhode Island as the foundation of an ever-improving integrated, accessible, affordable, and equitable health care system. CTC-RI brings together critical stakeholders to implement, evaluate and spread effective multi-payer models to deliver, pay for and sustain high-quality, comprehensive, accountable primary care.
