Complex Care Conversations is an interactive curriculum designed for small groups to increase clinician ease and effectiveness in approaching goals of care conversations with patients dealing with serious, progressive illnesses such as heart failure, cancer and dementia.
RI World Diabetes Day (RIWDD) was launched in 2019 by the RI Department of Health as a vehicle for raising awareness about diabetes and its impact on residents. The event exemplifies the strong collaboration between the many community organizations, state government officials, health entities, and community residents who are committed to decreasing the prevalence of diabetes.
The mission of CTC-RI is to support the continuing transformation of primary care in Rhode Island as the foundation of an ever-improving integrated, accessible, affordable, and equitable health care system. CTC-RI brings together critical stakeholders to implement, evaluate and spread effective multi-payer models to deliver, pay for and sustain high-quality, comprehensive, accountable primary care.
