50+ Nurse Care Managers participate in diabetes "Train the Trainer" program
CTC, in partnership with Healthcentric Advisors, provided Nurse Care Managers (NCMs) with an opportunity to complete a Diabetes "Train the Trainer" Program at the January 15th NCM Best Practice Sharing Committee meeting. Participants received a Diabetes Toolkit with resource materials developed by the Greater Providence Coalition's Medication Safety Workgroup.

Tufts Health Plan grants funding support to CTC for 2019
CTC is pleased to announce it has received generous funding from Tufts Health Plan for 2019 to support the following two efforts:
Health plans support PCMH-Kids practices
PCMH-Kids and CTC are pleased to announce that all the health plans have agreed to provide continued financial support for PCMH-Kids Cohort 1 practices.
Integrated Behavioral Health Practice Facilitation training program extended
CTC is proud to announce that, with funding from Tufts Health Plan, we have been able to extend the Integrated Behavioral Health (IBH) Practice Facilitator training program. This month, CTC has accepted three more applicants to become certified IBH Practice Facilitators.
