  • April 29, 2025: Diabetes Pharmacy Learning Collaborative - During our recent learning collaborative, practices highlighted pharmacy-led interventions to improve diabetes care, including data-driven workflows, and facilited discussions on tackling medication access barriers under new Medicare rules.  Practice members may reach out to Der Kue to obtain access to recording and slides.
  • April 30, 2025: CVD Learning Collaborative - During this recent Learning Collaborative, practice consultants shared updates on community health programs and data tools, and highlighted progress in using Z-codes and social needs screening to better support patients, while practice facilitators introduced care planning as the next focus area. Practice members may reach out to Der Kue to obtain access to recording and slides.
