- April 29, 2025: Diabetes Pharmacy Learning Collaborative - During our recent learning collaborative, practices highlighted pharmacy-led interventions to improve diabetes care, including data-driven workflows, and facilited discussions on tackling medication access barriers under new Medicare rules. Practice members may reach out to Der Kue to obtain access to recording and slides.
- April 30, 2025: CVD Learning Collaborative - During this recent Learning Collaborative, practice consultants shared updates on community health programs and data tools, and highlighted progress in using Z-codes and social needs screening to better support patients, while practice facilitators introduced care planning as the next focus area.
