The CTC Board of Directors is pleased to announce that it has approved the 2019 PCMH-Kids expansion program, including 17 practice sites, representing 20,000+ attributed lives. The Selection Committee conducted a rigorous review of all applications and came to a unanimous conclusion to accept these practices.
Rhode Island Quality Institute, with funding from the State Innovation Model, is subcontracting with CTC to launch an Advance Directive Learning Collaborative Network opportunity.
At the December 6th PCMH Kids Stakeholder Meeting Cohort 1 practices were recognized for their achievements as they complete the 3-year contract effective 12/31/18.
CTC is partnering with the American College of Physicians to sponsor a statewide learning collaborative for primary care and specialist offices to enhance High-Value Care Coordination. The program will start on or about late January and continue through July 2019.
CTC's Quarterly Breakfast of Champions meeting is being held on February 8th at 7:30am. This is an important point in time for primary care in Rhode Island and for the CTC and PCMH-Kids program, and the involvement and input from all CTC practices is critical. Specifically, all clinical practice champions and practice site clinical and administrative leaders should attend.
UnitedHealthcare is providing CTC with a funding opportunity of $148,000 to support the continued development of our statewide nurse care manager core curriculum workforce development program.
