CPC+ is ending and Primary Care First is starting up. Ready, Set, Apply! The Centers for Medicare & Medicaid Services (CMS) is announcing that the Comprehensive Primary Care Plus (CPC+) Model will end on December 31, 2021, for all model participants, regardless of start date.
On March 5, eleven practices met to kick off a new year-long IBH learning collaborative funded by UnitedHealthcare and the Rhode Island Foundation. Nelly Burdette, PsyD is leading the project, and Kristin David, PhD and Liz Cantor, PhD, will be serving as practice facilitators.
The learning collaborative will focus on two objectives:
The mission of CTC-RI is to support the continuing transformation of primary care in Rhode Island as the foundation of an ever-improving integrated, accessible, affordable, and equitable health care system. CTC-RI brings together critical stakeholders to implement, evaluate and spread effective multi-payer models to deliver, pay for and sustain high-quality, comprehensive, accountable primary care.
