Why did your practice first engage in the family home visiting program?
Sara Remington serves as the RI Department of Health's (RIDOH) Family Home Visiting Implementation Manager, helping implement, guide, and support important home visiting efforts statewide, including the Healthy Tomorrows program with pediatric primary care practices.
The RI Office of the Health Insurance Commissioner (OHIC) provides primary care practices with the opportunity to submit their quality measure performance data in order to be considered for RI Patient Centered Medical Home (PCMH) designation.
The December Clinical Strategy Committee (CSC) concentrated on national and statewide data integration strategies. Two experts presented on how to use data to inform population health and deliver actionable information to providers.
January’s Integrated Behavioral Health (IBH) meeting was focused on CTC-RI’s Pediatric IBH program. Liz Cantor, PhD reviewed project expectations and screening results, highlighting the tremendous performance practices achieved for adding and improving screenings for behavioral health conditions within pediatric primary care practices.
