First Cohort of Telehealth Learning Collaborative Practices Present Storyboards

The first cohort of Telehealth Learning Collaborative practices celebrated a year of hard work, achievements and learning at the Cohort 1 wrap up on January 27th. The thirteen practices have spent the last year focusing on projects addressing a range of chronic conditions, exploring the use of technology to better care for patients during and after the pandemic.

We encourage you to take a look at some of the PDSAs and lessons learned that these practices shared. The meeting also included a fifteen-minute presentation from Reid Plimpton of the Northeast Telehealth Resource Center, “Telehealth in 2022 and Beyond,” which was packed with resources to help Rhode Island primary care practices understand the telehealth landscape at the state and national level as we move into the new year.

The year-long learning Telehealth Learning collaborative has been funded through authorized CARES Act funding, UnitedHealthcare and Rhode Island Department of Health. We look forward to hearing from our eight Cohort Two practices at our April 27 Wrap up meeting.
