Practice clinical quality and customer experience performance was reviewed at the April Practice Reporting and Transformation Committee meeting. Practices are encouraged to review their 1st quarter 2019 clinical quality performance report for accuracy.
Integra Community Care Network, an ACO in partnership with Care New England, RI PCPC and South County Health, has engaged CTC services for a two-year contract to assist selected Integra practices with implementing fully-integrated care that will address the medical, behavioral, and substance use needs of patients in a coordinated, systemic, and unified way.
Please register and attend the CTC Quarterly Breakfast of Champions. All Clinical Practice Champions and practice site clinical and administrative leaders are expected to attend.
The Clinical Strategy Committee (CSC) is a working subcommittee of the CTC Board of Directors. CSC's purpose is to enhance the ability of CTC participating practices and related systems of care to achieve improved quality and reduced costs.
CTC has redesigned our practice secured portal! Upon successful login, practices can easily navigate the portal to either submit data, or view data displays, resource documents and announcements. Users can submit quality measures, high-risk reporting, and quarterly provider/care coordinator staff updates via links located in our "Data Input" section.
