UnitedHealthcare is providing CTC with a funding opportunity of $148,000 to support the continued development of our statewide nurse care manager core curriculum workforce development program.
Adult CTC practices will start reporting on the HEDIS 2019 controlling high blood pressure measure (blood pressure control defined as 140/90 for adults age 18-85) beginning 1/15/19 for timeframe 1/1/18-12/31/18. The previous measure (control defined as 140/90 for adults 18-59 or patient with diabetes 18-85; 150/90 for patients 60-85) has been retired effective 12/1/18.

We would like to thank all who attended to discuss and learn ways in which local primary care teams can identify, develop, and utilize comprehensive strategies to improve care and lower costs.
