Practice Reporting and Transformation Update

Under State Opioid Response (SOR) grant funding, a Family Care Liaison position at Rhode Island Parent Information Network (RIPIN) has been created to serve as a central partner and resource to Community Health Teams (CHT) and PCMH Kids practices. The Family Care Liaison will serve as an adjunct to the CHT to provide information, resources and connections related to the child(ren) to both families and professionals. The Family Care Liaison will assess the needs and eligibility of the child/children and identify and coordinate with appropriate programs to meet their needs. The Family Care Liaison will help strengthen coordination of care and reduce duplication of services in which the families are involved with. They will also serve as a resource to care coordinators at PCMH Kids practices to help build capacity and strengthen their knowledge. Families are eligible for referral to the Family Care Liaison regardless of health insurance type and can help any family with a child in need of in-home services, at risk for food/housing insecurities, overwhelmed with their child's needs, and/or is affected by Substance Use Disorder or at risk for Opiate Use Disorder.
