Pharmacy QI Learning Collaborative Meeting
Improving Population Health / Reducing Low-Value Care in Primary Care through Professional Use of Continuous Glucose Monitoring (Pro CGM)
Primary care practices that are part of systems of care and participating in the Pharmacy QI Learning Collaborative met in December to share their progress on the program identifying patients who might benefit from professional use of continuous glucose monitoring interventions. The pharmacy leader presented goals and team-based workflows that they’ll be instituting. Each organization is approaching the 2-year Pro CGM initiative in a different manner. Leslie High, Abbott Diabetes Care, provided a tutorial and answered questions on Libreview and how it can be used to customize practice reports. The meeting concluded with a discussion on billing options for Pro CGM services. Special thanks to UnitedHealthcare and RIDOH who are providing the funding for this important quality improvement initiative.
