Midpoint Learning Collaborative on Delivering Age-Friendly Care

Last month, CTC-RI held a midpoint Learning Collaborative as part of the Delivering Age-Friendly Care Implementing the 4M Model and Supporting Care Partners of People with Dementia project. This initiative supports primary care practices in adopting the 4M Model (What Matters, Medication, Mentation, and Mobility) to enhance care for older adults and their care partners while achieving Level 1 or Level 2 Recognition from the Institute for Healthcare Improvement. The session brought together 34 attendees, including representatives from five participating practices, who shared updates on their PDSA cycles. These updates focused on refining care partner education and improving access to resources. Participating practices included:

  • Breakwater Primary Care (Lincoln and Warwick locations)
  • Family Care Center
  • PACE RI
  • Brown Medicine
  • University Internal Medicine

Guest speakers included Aura Medina, The POINT Network Manager, and Robyn Earley, CareLink Chief Clinical Officer. Aura shared her expertise on long-term care services, including Medicare, Medicaid, and state and federal social service programs, while Robyn discussed the innovative CareLink programs to enhance post-acute and community-based care for older adults and adults with disabilities. This session is available to view along with access to session materials by clicking here. We extend our gratitude to UnitedHealthcare and the RIDOH for their generous funding and support of this important initiative.
