Providence Community Health Center (PCHC) and Brown Practice Facilitators Present at the PCMH Congress

Chris Camillo, Chief Operating Officer at PCHC, together with Suzanne Herzberg and Caitlin Towey (CTC/Brown Practice Facilitators) presented at the national 2017 NCQA Patient Centered Medical Home Congress conference.  Their break out session “PCMH Transformation and Collaboration in an Urban Multi-Site FQHC” provided attendees with tools, resources and strategies for transformation projects and offered examples for  using them  at different stages of clinical transformation.  Their thoughtful and well received presentation identified common barriers and solutions for identifying transformation leadership, engaging staff and providers, adopting PCMH change concepts, measuring outcomes and using practice facilitators to assist with transformation efforts.
