CTC June Clinical Strategy Committee Meeting

The June CTC Clinical Strategy Committee meeting included these most recent presentations: Stephen Kogut, PhD MBA RPh and Emily Patry, MS on “Empowering the Safe, Effective and Efficient (S-E-E) Use of Medication in Older Adults”; Dr. Alexis Lawrence on “Engaging High Utilizers: An Opportunity to Collaborate with Brown Emergency Medicine”; and Jessica Savoca, MBA, MPH on the Brown Medicine Patient Family Advisory Council (PFAC) Best Practice Sharing. Additionally, CTC is participating in a RIDOH task force to improve the transition of care process with a focus on improved delivery and format of the discharge continuity of care document (CCD). A working group is being organized for September to focus on hospital to community provider transition. The group will include hospital CIOs and leaders in care management, primary care, and behavioral health with a goal of providing concrete recommendations by October.
