Practices Invited to Participate in the 2022 Pediatric/Adult Primary Care Health Care Transfer of Care Quality Improvement Initiative
CTC-RI/PCMH Kids, in collaboration with Tufts Health Plan and the Rhode Island Department of Health (RIDOH) is pleased to announce that the following practices have been invited to join the 2022 Pediatric/Adult Primary Care Health Care Transfer of Care Quality Improvement Initiative.

- Hasbro Children’s Hospital Pediatric Primary Care
- Center for Primary Care
- Children’s Choice Pediatrics
- Greenwich Medical Associates
- Dr. Chad Lamendola
- Dr. Richard Ohnmacht
These practices include both new and continuation teams that will participate in a year-long learning collaborative. Together they will work on creating and refining an efficient and sustainable approach to assisting youth and young adults as they transfer from pediatric to adult care. Transfer activities will take into consideration youth and young adults with and without special healthcare needs. Participating practices will be attending a collaborative kickoff in June.
