NCM/CC Best Practice Sharing: Geriatric Education Series on Dementia
The Rhode Island Geriatric Education Center presented on “Dementia Care: Caring 4 Caregivers”, at the May NCM/CC Best Practice Sharing meeting. Maureen Maigret, Aging and Long-Term Health Care Policy Consultant, shared the challenges and stress factors experienced by caregivers, and provided information on available programs and resources, including the Caregiver Assessment from the RI Office of Healthy Aging. Michelle Szylin, Associate Director RI Office of Healthy Aging, and Linnea Tuttle, Chief of Family Health Services, RI EOHHS, provided information regarding available self-directed programs, community programs and resources available. Learners were eligible to receive CME/CEU’s.
The June NCM/CC meeting will share best practices for a successful pediatric to adult transition of care. The National Alliance to Advance Adolescent Health will share data and lessons learned from implementing health care transitions. Practices that participated in the first health transitions learning collaborative will share their experiences.
