Practices Selected to Participate in the 2021 Pharmacy Quality Improvement Initiative
The CTC-RI and RIDOH in collaboration with faculty from URI College of Pharmacy is pleased to announce that the following practices have been selected to join the 2021 Pharmacy Quality Improvement Initiative: Reducing Preventable Hospitalizations and Emergency Department Usage through Team-Based Care.

- Coastal Medical East Greenwich
- Medical Associates of Rhode Island
- Thundermist Health Center
- Providence Community Health Center
- Rhode Island Primary Care Physicians
- Women’s Medicine Collaborative
These 7 practices will participate in a pharmacy-led, team-based care initiative to provide primary care practices with an interprofessional quality improvement learning opportunity with the aim of reducing preventable hospital care for emergency department (ED) use and/or inpatient admissions. The Prevention Quality Indicators (PQIs) were developed by the federal Agency of Healthcare Research and Quality (AHRQ) to measure rates of preventable hospital care for a range of "ambulatory care sensitive conditions." Practices will use this baseline information to identify areas of clinical focus that are specific to their practice/SOC and develop and implement team-based pharmacy performance improvement plans.
