Community Health Team Expansion May Best Practice Sharing Meeting

In May, the Community Health Team (CHT) Expansion project, funded by RIDOH, brought together South County Health and East Bay Community Action Program practice teams for a best practice sharing meeting. We heard updates from both practice teams on their data outcomes, their successes and barriers to success in the CHT Expansion project. The meeting also included a training session from Tonya Glantz, PhD, from Rhode Island College’s Institute for Education in Healthcare, on Threats to Healthy Aging: Behavioral Health Changes.
