Call for Applications: “Implementing Primary Care and Pharmacy Strategies to Improve Care for Patients with Diabetes”
CTC-RI is excited to announce the new project and Call for Applications for the “Implementing Primary Care and Pharmacy Strategies to Improve Care for Patients with Diabetes” project. The goal of this two-year learning collaborative is to improve screening rates and management for patients with diabetes for earlier detection of diabetes-related eye of and kidney disease. Our pharmacy practice facilitator will help practices in meeting quality improvement expectations to improve their screening and treatment of patients with diabetes and to provide earlier detection of retinopathy and nephropathy.
Over the two year time period, practices will be eligible to receive up to $20,000.
The project is a partnership with University of Rhode Island (URI) College of Pharmacy and Rhode Island Department of Health (RIDOH), with funding provided by United Healthcare and RIDOH.
