Best Practice Sharing: Practice Reporting and Transformation Committee Meeting

Adult practices (Cohort 5) and PCMH Kids practices (Cohort 2 and 3) met on April 22 to discuss clinical quality and customer experience (CAHPS) outcomes, like successful pediatric developmental screening shown below. This year, in recognition of the significant strain practices were experiencing with managing patient needs during COVID-19, practices in Adult Cohort 5 and PCMH Kids Cohort 2 were provided the opportunity to use quarter 4 results for contract adjudication. Significant to note, practice performance for both adult and pediatric practices improved over the three-year period they were participating in the CTC-RI/PCMH Kids program for both outcome and process clinical measures and for communication and office staff customer experience measures.
