CTC and PCMH-Kids Portal Redesign

CTC has redesigned our practice secured portal! Upon successful login, practices can easily navigate the portal to either submit data, or view data displays, resource documents and announcements. Users can submit quality measures, high-risk reporting, and quarterly provider/care coordinator staff updates via links located in our "Data Input" section. Practices can visit our "Data Display" section to view cohort and practice-level performance over time via performance dashboards, clinical quality and patient experience data views, and CAHPS report documents. There are also sections for practice reporting resource documentation and special announcements. For users who already have login credentials, those are still valid to enter the portal. For those who are lacking login credentials, CTC project management staff will provide new user permissions. It is important to note that CTC secured portal information is intended for use only by members of the RI CTC Community Program.
