CTC-RI launched a new Project ECHO® Program series in March: “Approaches to Dementia Care: Building Blocks for Becoming Age-Friendly”. Our 1st session, “Better Patient Care with an Age-Friendly Approach”, presented by Ana Tuya Fulton, MD, FACP, AGSF, provided the 30+ participants with strategies to help learners deliver care aligned with the age-friendly approach. Ashley Butler, RN, BSN from St. Elizabeth’s Home, presented a case study which provided an opportunity for applied learning, discussion and recommendations. Future sessions will include training on functional status assessments, safety screenings, understanding and responding to behaviors associated with dementia, and support for dementia caregivers. The 6-session series is intended to assist primary care practices and other organizations move toward adopting an age-friendly approach based on the 4M Framework: What Matters Most, Medications, Mentation and Mobility and has been made possible with funding from UnitedHealthcare, RIGEC and RIDOH. To view the presentation slides from the meeting, click the link here.
