March Clinical Strategy Meeting on Building an Integrated Delivery System

CTC-RI’s March Clinical Strategy Committee meeting shared strategies for building advanced primary care / integrated delivery system by looking at successful models from TX, PA and NY. Scott Conard, MD, Converging Health, talked about recognizing the importance of culture change and leadership training to support primary care capitation implementation while using data-driven workflows to support team-based care.
