CTC-RI Showcases Success of proCGM Quality Improvement Project at AMCP 2025

CTC-RI's highly successful quality improvement project to expand the use of professional Continuous Glucose Monitoring (proCGM) in primary care was presented at the Academy of Managed Care Pharmacy (AMCP) 2025 Annual Meeting on April 2nd in Houston, Texas.
Spearheaded by Dr. Kelley Sanzen, this pharmacist-led initiative involved over 400 patients who used proCGM, followed by consultations with an ambulatory care pharmacist to review results and implement lifestyle and medication adjustments as needed. Within 3–6 months, participating patients achieved lower blood glucose levels. Additionally, survey data showed that the program had a highly favorable impact on care team well-being.
