March CTC-RI Best Practices in Team-Based Care

In March, we had an engaging Best Practices in Team-Based Care session: "Lessons Learned: Integrating Community Health Worker Billing into Pediatric Primary Care”. Our expert panel shared lessons learned and strategies for having the clinical, IT and billing staff work together to design and implement templates and workflows to bill for community health worker services:

  • How CHW billing can be integrated into pediatric primary care workflows to support CHW sustainability. 
  • Collaborative team efforts that can be used to establish CHW billing processes. 
  • Successes and challenges from practices using the DULCE model to integrate CHW billing in pediatric and family care settings.
