Delivering Age-Friendly Care Final Learning Collaborative: Implementing the 4M Model and Supporting Care Partners of People with Dementia

In February, CTC-RI hosted the final learning collaborative session for the "Delivering Age-Friendly Care" project. During this session, participating practices shared their progress, including PDSA cycles and case studies, and noted their progress in summitting applications for Institute for Healthcare Improvement (IHI) Level 1 or Level 2 Age-Friendly Recognition. This session marked the conclusion of a six-month quality improvement initiative designed to support practices in implementing the 4M Ambulatory Care Age-Friendly Model and enhancing care for older adults and individuals living with dementia. Practices also focused on identifying and supporting care partners to ensure that they received appropriate education and resources, based on what mattered to them. Participating Practices: 

  • Brown Medicine 
  • University Internal Medicine 
  • Breakwater Primary Care (Drs. Yamada and Steinmetz) 
  • CNE Family Care Center 
  • PACE-RI 

Throughout the collaborative, practices worked diligently to integrate the 4M Framework (What Matters, Medication, Mentation, and Mobility) into their workflows. At the final session, practices shared results from the successful implementation of their performance improvement plans, case study success stories, and lessons learned.
