Care Transformation Collaborative of Rhode Island (CTC-RI) Selection Committee Announces the Appointment of Nurse Care Manager/Care Coordinator (NCM/CC) Faculty Positions
Experienced Nurse Care Managers/Care Coordinators (NCM/CC) were invited to apply for NCM/CC faculty positions whose role will be to provide coaching support for new and experienced NCM/CC and assist them with developing skills in supporting patients/families that have complex care needs.
CTC-RI formed a Selection Committee charged with reviewing faculty applications, interviewing and selecting candidates who were interested in pursuing this exciting career development opportunity. The Selection Committee received a number of very qualified NCM/CC applications and final selection was additionally based on factors such as expertise needed, geographic location, representation from various accountable care organizations/accountable entities, and representation from different practice types (i.e. PMCH-Kids, practice size).
CTC-RI is pleased to announce the following professionals have been selected and accepted positions as NCM/CC faculty:
Gloria Rose RN, Certified Diabetes Outreach Educator, Director of Community Care Thundermist Health Center
Jayne DaylorRN, MS (Psychiatric Nursing) Yellow Belt Lean Training, Practice & Quality Manager South County Health System
Kateri Chappell, RN, BS Nurse Care Manger with experience as Infusion Nurse and home care, NCM Coastal Medical
Linda Hughes, Care Manager/Social Worker with experience as pediatric care manger and working with children with disabilities, Care Manager East Greenwich Pediatrics PCMH Kids Practice
Karolyn McKay, RN, NCM, Certified Diabetic Educator, NCM University Medicine
Carol Falcone, RN, Certified APPHRN with home care experience;Adjunct faculty at RIC, NCM supervisor since June 2015 at East Bay Community Action (Adult and PCHM Kids practice site);
Reena M. Jariwala, RN, BSN CCM Senior Nurse Care Manager with health plan experience as care manager, NCM for Internal Medical Partners (Charter Care practice) for 3 years
Faculty will complete a specialized “Train the Trainer” program offered by the xG Learn education and design experts and become certified as xG Learn educators. Once the faculty members have successfully completed the faculty certification process, CTC-RI will begin offering NCM/CC the opportunity to complete NCM/CC core curriculum training modules. Faculty will be assigned to work with a cohort of 7-10 fellow NCM/CC and learn how to apply care management skills to “real life” situations to facilitate optimal patient outcomes.
