CTC Hosts 2nd Advance Directive Collaborative Meeting: "Advance Directive Planning: Who, How, What and Why"
Jennifer Ritzau MD, Medical Director/Director of Palliative Care Hope Hospice, was the featured content expert speaker at the 2nd Advance Directive Collaborative meeting held on 2/21/19 for the 10 practices/organizations that are participating in the State Innovation Model (SIM) funded initiative. Dr. Ritzau provided information on the advance directive documents that are used in Rhode Island (MOLST and Durable Power of Attorney forms) and provided suggested "Conversation Starters" for patients that are healthy adults, or have acute or chronic conditions or are seriously ill. Included in the presentation was a Communication Guide, included billing codes, that can be used for advance directive conversations.
Peggy Menna, RIQI Senior Trainer, and April Arnold, RIQI Project Manager, provided updated guidance (Provider Overview Frequently Asked Questions; Consumer Overview: Frequently Asked Questions) on using "KnowMyHealthRI", the platform that is being used to upload advance directive information. Participating practices/organizations reported out on their performance improvement plans to increase practice usage of the platform.
The Learning Collaborative sessions are open to other primary care practices/organizations with the next meeting being held on April 18 at Rhode Island Quality Institute (7:30 to 9:00am).
