February Clinical Strategy Meeting Focuses on LAN Social Risk Adjustment and Community-Based Platforms
Our February Clinical Strategy Committee meeting focused on LAN social risk adjustment & community resource platforms. Craig Jones, MD, Primary Care Collaborative, presented on the Health Care Payment Learning & Action Network’s (HCPLAN) creation of the Health Equity Advisory Team (HEAT) to advance health equity. The LAN established the HEAT to help identify and prioritize opportunities to advance health equity through APMs, to influence design principles, and to inform LAN priorities and initiatives. The presentation focused on defining effective social risk adjustment and actions for care transformation, data collection, and payment. Garry Bliss, Program Director, Prospect Medicaid AE, then presented on Prospect’s experience leveraging Unite Us for community referrals while Charlie Estabrook, MPH, EOHHS Director of Health System Transformation, provided insight into platform sustainability. Larry Warner, DrPH, United Way Chief Impact and Equity Officer, then spoke about United Way’s partnership with Blue Cross Blue Shield to expand access and resources. Craig Jones, MD closed the meeting sharing experiences and lessons learned from Idaho on the integration of HIE and a social needs platform.
