Practice Reporting and Transformation Committee Update

Adult Practices (Cohort 5) and PCMH Kids (Cohort 2) reported out on 4th quarter clinical quality performance and PCMH Kids (Cohort 2) additionally reported out on NCM/CC engagement with at-risk patients/families. Practices are encouraged to review their most recent clinical reports to determine if performance meets performance targets either by Method 1 (meets target) or by Method 2 (improvement method). The next clinical quality reports will be due April 15th and this information will be used to determine practice eligibility for incentive payments (click here for Adult Performance Standards and here for PCMH Kids Performance Standards). PCMH Kids Cohort 3 practices reported on their baseline performance. Practice clinical quality performance information has been posted to the CTC secure portal. 

OHIC has decided to delay implementation of the updated measure: clinical depression and follow-up and more information will be forthcoming at future meetings.

Important to note: NCQA has updated its Annual Reporting requirements. Suzanne Herzberg PhD, OTR/L, Jayne Daylor RN MSN (CTC-RI Practice Facilitators) and Bernadette Parrillo, BSRRT, CBET-E (RIPCPC Quality Improvement Specialists) provided information on the NCQA Annual Reporting requirements including recent changes, effective January 1, 2020.
