Community Health Team Evaluation Efforts Underway

Community Health Team (CHT) evaluation efforts are underway. Evaluators from University of RI have examined available CHT intake and discharge scores and reported an average intake score of 16.4 and an average discharge score of 9.3 (43% decrease), a statistically significant result indicating improved outcomes for CHT patients.  This is with an average length of stay in the CHTs of 7 months. Data collection efforts are still underway and more detailed outcome data will be available soon.
