CTC Hosts Annual Large Learning Collaborative: “Primary Care Works: Effective Strategies to Improve Population Health and Patient Activation”

Over 300 people attended the 2017 CTC Annual Learning Collaborative which featured national, regional, and local experts speaking on topics to assist primary care practices with using practical and evidence based approaches to improve health outcomes through better utilization of team based care and patient activation strategies.  The keynote address was given by Peter Anderson MD, founder and President of Team Care Medicine.  His address spoke about the need for primary care practices to provide better access for patients with urgent needs and offered team based strategies that have been demonstrated to improve access, quality outcomes, patient, provider and team member satisfaction.  There were a series of breakout sessions with learning tracks for people interested in topics designed to provide learning opportunities for improving care for children and families, patients with substance use disorders, and patients with chronic and complex care needs.  Sessions were also offered on increasing patient activation and using data to improve performance.   Attendees were additionally invited to learn more about adult and pediatric community resources by visiting the 20+ exhibitor tables.  Links to all the presentations can be found on the CTC-RI website by clicking here.  Many thanks are extended to our sponsors  and planning committee members who helped make this primary care event possible.
