The CTC-RI Annual Conference, Advancing Primary Care: Innovating for a Sustainable Future, will take place on November 19, 2025, at the Crowne Plaza in Warwick, RI.   • Brown Health Medical Group Primary Care will present on "Transforming Primary Care: Addressing Workforce Shortages and Sustainable Solutions." Additional speakers from Brown University Health will discuss "Expanding and Improving Medicare Annual Wellness Visits in Primary Care," with a focus on aging and older adult health.
  • CareLink will present a session focused on aging adults and their families, titled "Effectively Training Primary Care Physicians to Screen and Refer Patients with Intellectual and Developmental Disabilities and Dementia."

  • Legal Key Partnership for Health and Justice with The Rhode Island Department of Health Office of Family Visiting discuss "Adding Legal Problem-Solving to the Home Visiting Toolbox: Lessons Learned"
  • The Rhode Island Department of Health partners with Brown University School of Public Health to share "Forming the Providence Community Advisory Network (Providence CAN): Community preferences for hypertension management and diabetes prevention"
