Care Coordination First ECHO Learning Session – Entitlement to Eligibility

In May, CTC-RI hosted the first Care Coordination ECHO session on Entitlement to Eligibility with funding and support from the Rhode Island Department of Health. This was the first session of our seven-session series that focuses on strategies to improve care coordination for families with young adults with special healthcare needs transitioning from pediatric to adult care.

Our RIPIN presenters, Tara Hayes, Family Voices Manager and Stephanie Trafka, Family Voices Health Research Support Specialist provided a case presentation that highlighted challenges that families experience in transitioning from pediatric to adult healthcare services. They also facilitated an open discussion about opportunities for improvement in the transitions process, outlined how partnering with RIPIN can help families and professionals navigate this process together, and demonstrated how the Medical Home Portal can be used as a resource. Fifty-nine participants attended live to improve their understanding of the process of entitlement to eligibility, share their experiences, and ask questions.
