December Care + Community + Equity Best Practice Sharing

The Care + Community + Equity (CCE) program held a best practice sharing meeting to hear presentations from participating practices on their projects to improve pre-diabetes and diabetes care delivery. Clinical teams from Tri-County Community Action Program and Comprehensive Community Action Program (CCAP) presented on their progress made in this CCE incentive area. We heard from Casey Sardo presenting for Tri-County, and Dr. Jason Villa, Assistant Medical Director, presenting for CCAP.

In addition to these practice presentations, meeting attendees heard from Benvinda Santos from RIDOH and Krystal Bevilacqua from Coastal Medical, who introduced the Medicaid Beneficiary Enrollment Pilot (MBEP) program. The CCE teams also learned about the CODAC Behavioral Health Mobile Unit from Alayna Woodley. Our next CCE Best Practice Sharing meeting will occur in March 2023.
