Rhode to Equity Teams Work on Upstream Causes of Inequity
In March, the 6 Rhode to Equity Teams came together for Momentum Session facilitated by the Wellbeing and Equity in the World Team (WE Team), provided teams with working time to build out their action plan for year 2 of the Rhode to Equity project, with a focus on the 3rd and 4th portfolios of the Pathways to Population Health framework. Portfolio 3 is community health and wellbeing while portfolio 4 is community solutions (work that addresses underlying causes of inequity).
To add their expertise to the mix, 2 subject matter experts were in attendance: Brian Rahmer, senior fellow at WE in the World, contributed to a breakout on housing as Hoda Abdullahi of Living Well Kent shared her experience in food access and equity.
The Rhode to Equity project kicked off in July 2021 and will move into its 2nd year this summer. The effort centers around the Pathways to Population Health framework, bringing together 6 teams made up of a health equity zone (HEZ) as well as other partners in the work: accountable entities, primary care clinics, community health teams, community-based organizations, and individuals with lived experience of inequities.
