CTC –RI/PCMH Congratulates the Nurse Care Managers Who Completed the GLearn NCM Core Curriculum Program
Please join us in congratulating the most recent group of Nurse Care Managers who completed the GLearn NCM Core Curriculum! We had 48 learners go through the 12-week program this year, representing every system of care in the state. The program culminated in learners preparing a capstone presentation and sharing their work at our NCM/Care Coordinator Best Practice Sharing meeting. The 2021 cohort of learners marks the fourth year CTC-RI/PCMH Kids has been able to offer this program, which has been made possible through the generous funding from UnitedHealthcare and the RI Department of Health. In 2018 CTC-RI/PCMH Kids responded to the need that practices voiced for NCM workforce development by partnered with Geisinger to offer the Nurse Care Manager Core Curriculum program. Geisinger offers an online modules covering disease-specific content as well as patient centered medical home care management concepts and patient engagement skills. Each module is supplemented by weekly group discussion calls facilitated by Rhode Island NCM faculty who have successfully completed the GLearn faculty training program.
