Primary Care First Update: CTC-RI Hosts Meeting with Nicholas Minter, CMMI Director of the Division of the Advanced Primary Care, Seamless Care Models Group

Rhode Island is one of 26 regions in the country that can participate in Primary Care First, a 5-year alternative payment model designed to reduce Medicare spending by preventing avoidable inpatient hospital admissions and improve quality and access to care, particularly for patients with complex chronic conditions and serious illness. Nicholas Minter was invited to Rhode Island to provide an overview of both Primary Care First and the Seriously Ill Population models of care and answer questions to assist our primary care community with learning more about this important opportunity. Primary care First has recently published a newsletter that includes important information for primary care practices on how and when to apply.

There are currently two publicly available calculators to simulate practice experience in Primary Care First. The first calculator was released by HealthTeamWorks, one of the CPC+ learning subcontractors in Colorado. The second calculator was developed internally by the American Academy of Family Physicians.
