Community Health Team Oversight Committee Meeting Recap

Christopher Ausura, Health Equity Zone Project Manager, presented at the Community Health Team Oversight Committee Meeting on December 6 to discuss enhancing connections between CHTs and Health Equity Zones (HEZs). Many social and environmental factors contribute to the health of a community and the mix of these factors vary widely from one community to another. Chris discussed the challenges to address these issues in diverse communities and how CHTs and HEZs can innovative and collaborate to reduce health disparities and improve health outcomes. Nicholas Minter, Director, Division of Advanced Primary Care, CMMI, also joined the meeting and discussed the feasibility for the CHTs to support Medicare Patients under the Seriously Ill Population (SIP) payment model.
