Congratulations to the Adult Start up Practices (click here for the full list) that successfully progressed to the next phase in the CTC Developmental Contract: Transition. With completion of the Start-up deliverables and submission of the nurse care manager reports these practices are
CMS opened the application for practices to apply for Comprehensive Primary Care Plus (CPC+), the largest-ever initiative to improve primary care in America.
PROVIDENCE – Rhode Island’s primary care practices are invited to participate in a national primary care medical home model, effective January 2017, according to the Centers for Medicare & Medicaid Services. The Comprehensive Primary Care Plus, a five-year medical home model, enables primary care practices to choose how to deliver the best outcomes and to receive payment for achieving results and improving care. Through CPC+, practices of diverse sizes, structures and ownerships are eligible to seek qualification in CMS’ Quality Payment Program’s Advanced Alternative Payment Models’ incentive payments. Primary care practices may apply to participate in CPC+ between Aug. 1 and Sept. 15.
CTC is excited to announce the official launch of the Integrated Behavioral Health Pilot Program.
RIQI has developed Care Management Dashboards which support near real time information about patients who are in or have recently been discharged from the hospital or emergency department. RIQI is pleased to offer CTC practices the opportunity to apply to participate in the Care Management Dashboard project funded by Rhode Island Foundation. Through this funding, RIQI will provide dashboards for up to 10,000 high risk patients across up to 30 practices.
