For April’s webinar momentum session, the WE Team invited David Waters, MA, Chief Executive Officer of Community Servings, to present on the work they are doing to actively engage the community to provide medically tailored, nutritious, scratch-made meals to chronically and critically ill individuals and their families.
During the month of April, Community Health Teams and SBIRT screeners had the opportunity to participate in 2 unique trainings. On April 14, Jope Consulting Services hosted Part 2 of an online Motivational Interviewing (MI) training. Twenty-two attendees were able to enhance their skillsets associated with supporting individuals facilitate behavior changes.

Thank you to everyone who joined us for the 4th installation in our "Advancing Team-Based Telehealth in RI" webinar series. The March 23 webinar focused on strategies for community health workers (CHW) and patient navigators to help their clients successfully utilize telehealth.
Northeast Telehealth Resource Center (NETRC) featured CTC-RI in its January Field Report: “Sample Stakeholder Challenges, Solutions and Lessons Learned.” The report highlighted our collaborative work in the 3 phases of our UnitedHealthcare and CARES Act funded Telehealth project: 1.
RIDOH in partnership with CTC-RI/PCMH Kids successfully applied for a 5-year HRSA “Healthy Tomorrows” grant whose goal is to improve communication and care coordination between the Family Visiting Program (FV) and Pediatric Primary Care Practices (PCMH Kids) with a specific focus on improving well-child care.
The mission of CTC-RI is to support the continuing transformation of primary care in Rhode Island as the foundation of an ever-improving integrated, accessible, affordable, and equitable health care system.
