CTC-RI is excited to launch a new application for the spring session of the UMass Chan Medical School Certificate Course in Primary Care Behavioral Health.
With generous funding support from UnitedHealthcare, Rhode Island Department of Health and Blue Cross & Blue Shield of Rhode Island, CTC-RI will launch several innovative projects in 2023 to invest in and strengthen primary care capacity in several priority areas. Anticipated projects in the new year include:
CTC-RI’s December 2022 Clinical Strategy Committee meeting focused on the role of pregnancy intention in improving birth outcomes and access to same-day LARC.
The November Health Transitions of Care learning collaborative had 16 participants join to listen and share patient and family experience, available resources, and best practices around what is helpful in improving care coordination for youth that are transitioning from pediatric to adult care.
Improving Population Health / Reducing Low-Value Care in Primary Care through Professional Use of Continuous Glucose Monitoring (Pro CGM)
The mission of CTC-RI is to support the continuing transformation of primary care in Rhode Island as the foundation of an ever-improving integrated, accessible, affordable, and equitable health care system. CTC-RI brings together critical stakeholders to implement, evaluate and spread effective multi-payer models to deliver, pay for and sustain high-quality, comprehensive, accountable primary care.
